Bring complete insurance/worker's compensation information to your appointment. If required by your insurance company or health plan, contact them prior to your scheduled appointment to verify coverage or obtain pre-authorization approval. Please be aware that although the MRI Centers have facilities on the campuses of Memorial Hospital and Saint Joseph Regional Medical Center - Mishawaka and Plymouth campuses, the MRI Centers is a separately owned and operated entity and DOES NOT participate with any of these hospitals billing plans or provider affiliations.

The MRI Centers will assist with insurance predetermination verification as a courtesy to the patient when it is requested in advance and allowed by the insurance carrier. Any predetermination information received from the insurance carrier is believed to be accurate. However, the MRI Centers takes no responsibility for the final outcome of coverage by the insurance carrier and expects the patient to pay any portion of their bill that is not reimbursed by their insurance carrier.

It is important when calling the insurance company that "Northern Indiana Magnetic Resonance Imaging Centers" is given as the name and NOT the hospital name.
